It seems somewhat reasonable in urbanized areas. I was able to get along without a car as a teenager in the 90s (in a town of 5000), although having one would have made my commute a lot shorter.

The chain that matters in this case is President > Secretary of Defense (Hegseth) > Commander, US Strategic Command (Admiral Correll), and there is probably some skip-echelon capability to allow for authorization of launch under attack.

Specifically to the point of the author’s dismissal of human error—proper root cause investigation goes beyond that. For example, “pilot error” is often further refined; something like “pilot error because the training program did not adequately address stall recovery.” The distinction is important; this type of attribution allows for actionable remedies vs just being the equivalent of “pilots need to get good.”

@munin@infosec.exchange The flight safety culture was there before the turn of the century, but yes this reads as very much in line with the medical community’s perspective at the time. I still don’t think they’ve done anything to address the fact that tired doctors make more mistakes (at least, not in the US), but it’s outside my expertise.
