I have found this in a couple of non-healthcare areas. When you follow the thread up the chain, that isn't what it said...or worse, the citation dies because the citation has nothing to do with the sentence making the claim.
In one case, I was not getting test results consistent with the results expected in the paper I was using. So, I was forced to go up the chain. At the last citation in the chain, the referenced paper had nothing to do with the sentence in the citation. I can't believe it was peer reviewed.
