So, I realised this morning that the horizontal timings of PAL vs NTSC are the same, it is just the different number of vertical lines that makes the difference in the refresh rate. So actually, I still don't really know what the issue was, but my updates to the code seem to make it a lot more reliable for me. Maybe my 2600's clock is a bit off. Or maybe the overclocking of the Pico is slightly different. Anyway, it works and that is the main thing, and I've learned a lot getting this far.
